General Strategy
Phase Rush is the biggest counter, as it gives 75% slow resistance. Blade of the Ruined King rush is recommended to steal movement speed and shred his high HP. Play aggressively 1-5, denying his stacks. Your
E cancels hisQ windup.
Tenacity (E. g. , Merc Treads) alone is bad against Wither; seek slow resistance (Phase Rush, Unflinching, Swiftness Boots).
Trading Patterns
When he runs at you, let him get as close as possible, then E him to cancel hisQ and maximize your knockback distance. If he uses Wither, immediately activate Phase Rush, proc BoRK, and run. Use Q (Blind) to prevent him from using Wither.
Wave Management
Stay on the outside of the wave so he must choose between farming with E or poking you. Force a deep wave freeze near your tower against passive Nasus players who hide under tower; this denies him all stacks.
