Victory in this matchup depends on maximizing your sustainability and resilience against Heimerdinger's severe burst damage. This is achieved by prioritizing the second max of 
W (Indestructible), thereby enhancing your sustain capabilities. Your essential item spike is the combination of Mercury Treads (Mercs), Cosmic Drive, and Spirit Visage (Spirit Vis), which significantly improves your capacity for short trades and sustain. Strategically, you should side lane more than team fight against him, utilizing the raw sustain from your W to eventually out-sustain his resources.
Heimerdinger Win Condition
Heimerdinger’s greatest strength is his insane burst damage, which is activated if Mordekaiser is caught in his grenade stun. If you are struck by the stun while he has all three turrets placed nearby with his
W active, you will be hit by an enhanced damage proc from the turrets followed by a full volley of rockets. This sequence will either massively chunk your health or result in an early game death. If Heimerdinger utilizes the Barrier summoner spell, his early game safety is significantly increased.
General Strategy
The optimal strategy requires adjustments to your runes: taking Absorb Life and Overgrowth. These runes are special because they are designed to stack off killing turrets: Absorb Life provides health whenever a turret is killed, and Overgrowth treats any nearby dead turret as a unit viable for stacking up the rune. Therefore, it is critical to always try to kill his turrets first before attempting any engagement on Heimerdinger himself. You can utilize your ultimate (R) to manage his setup: your ultimate ensures that all of his regular turrets remain outside the Death Realm.
If Heimerdinger uses his enhanced ultimate turret before you use your
R, the enhanced turret will also stay outside; however, if he places the ultimate turret inside the realm, he keeps it, but it will vanish once the Death Realm concludes. It is advisable to focus on side laning rather than team fighting, as Heimerdinger can easily "one tap" squishy targets in team fights by cheesing from a brush. Eventually, if you maintain pressure, Heimerdinger will run out of mana, limiting his ability to place new turrets or clear the wave, which is the perfect window to hit the tower or dive him.
